What happened
According to a notice filed with the California Attorney General, 5Star Life Insurance Company's benefits administrator, Paylogix, experienced a disruption to its network in November 2025. An investigation found that unauthorized actors viewed or removed certain files from the Paylogix environment between November 13, 2025 and November 18, 2025. Paylogix secured the affected systems, launched a forensic investigation, and notified law enforcement.
After restoring its systems, Paylogix reviewed the affected files one by one to identify the data involved and match each record to the correct customer organization. That review concluded on or around July 20, 2026, and Paylogix then notified the insurers whose data was involved, including 5Star Life Insurance Company. 5Star Life Insurance Company filed its notice with the California Attorney General on September 24, 2026, roughly ten months after the incident.
