What happened
On June 12, 2026, Baystate Health's Compliance and Privacy Office was notified that an employee at Baystate Medical Center, a Springfield, Massachusetts hospital, may have inadvertently placed one patient's admission page into another patient's discharge documents. Baystate investigated and determined the same day that another patient had received a copy of the admission sheet in error. The receiving patient returned the document to Baystate.
Baystate mailed notification letters on or around July 15, 2026 and reported the incident to the Massachusetts Attorney General on July 20, 2026. The hospital states it retrieved the misdirected document from the unintended recipient and is offering a complimentary two year (24 month) membership in Experian IdentityWorks Credit 3B, a three bureau credit monitoring service, with an enrollment deadline of September 30, 2026.
According to the notice, the misdirected admission sheet listed the affected patient's name, date of birth, and Social Security number, along with the date of service and hospital location. Because a Social Security number was involved, the disclosure carries a risk of identity theft even though it involved a single misdirected document. Whether Baystate's document handling procedures met applicable standards under HIPAA and Massachusetts data protection law is among the issues being evaluated.
