What happened
According to a filing with the Maine Attorney General, Florida Physician Specialists, a Jacksonville-based physician practice, detected unauthorized access to its computer network that occurred between November 27 and November 29, 2025. The practice retained outside cybersecurity professionals to investigate and determine the scope of the compromise. A subsequent document review concluded on April 6, 2026 confirmed that certain personal records were removed from the network.
Florida Physician Specialists began mailing notification letters on or around April 25, 2026, roughly five months after the breach window. The notice states that exposed data may include full names, Social Security numbers, driver's license or state identification numbers, other government identification numbers, financial account information, credit or debit card information, medical information, and health-insurance policy information. A separate filing with the Texas Attorney General reported that 319 Texas residents were among those affected, the total number of impacted individuals nationwide has not been publicly disclosed.
The practice is offering complimentary enrollment in Equifax Credit Watch Gold to affected patients. Whether the practice's pre-breach security practices met legal and HIPAA standards, and whether the notification delay caused additional harm, are among the issues being evaluated.

Keep your notice letter
