Background

What happened

According to a filing with the Maine Attorney General, Florida Physician Specialists, a Jacksonville-based physician practice, detected unauthorized access to its computer network that occurred between November 27 and November 29, 2025. The practice retained outside cybersecurity professionals to investigate and determine the scope of the compromise. A subsequent document review concluded on April 6, 2026 confirmed that certain personal records were removed from the network.

Florida Physician Specialists began mailing notification letters on or around April 25, 2026, roughly five months after the breach window. The notice states that exposed data may include full names, Social Security numbers, driver's license or state identification numbers, other government identification numbers, financial account information, credit or debit card information, medical information, and health-insurance policy information. A separate filing with the Texas Attorney General reported that 319 Texas residents were among those affected, the total number of impacted individuals nationwide has not been publicly disclosed.

The practice is offering complimentary enrollment in Equifax Credit Watch Gold to affected patients. Whether the practice's pre-breach security practices met legal and HIPAA standards, and whether the notification delay caused additional harm, are among the issues being evaluated.

FAQ

Common questions

I received a data breach letter from Florida Physician Specialists. What should I do? +

Keep the letter, activate the free Equifax Credit Watch Gold service using the instructions provided in your notice, place a fraud alert or freeze on your credit file with all three bureaus, and contact a data breach attorney for a free consultation. Because medical and health-insurance information was involved, also watch for unfamiliar Explanation of Benefits letters or insurance billing you do not recognize. Accepting credit monitoring does not waive your right to sue.

Am I eligible to join a class action against Florida Physician Specialists? +

If you received an April 2026 breach notice from Florida Physician Specialists, you are likely eligible for a free case evaluation. Eligibility depends on your state of residence, the categories of your data that were exposed, and any losses you have suffered. Dapeer Law will review your notice at no cost.

How much money could I receive from a class action lawsuit? +

Data breach class action recoveries vary significantly. Settlements typically range from a few hundred dollars for basic out-of-pocket losses to several thousand dollars for documented identity theft, with class size, damages, and negotiation all affecting the final amount. Healthcare breaches involving Social Security numbers and medical records often command higher per-class-member recoveries because of the sensitivity of the data. No payout is guaranteed, and this investigation has not yet resulted in a settlement.

What personal information was exposed in the breach? +

According to the Maine Attorney General filing, the data potentially exposed includes full names, Social Security numbers, driver's license or state identification numbers, other government identification numbers, financial account information, credit or debit card information, medical information, and health-insurance policy information. Your individual notice letter should specify the categories of your data that were involved.

Did Florida Physician Specialists offer free credit monitoring? +

Yes. The practice is offering complimentary enrollment in Equifax Credit Watch Gold, which provides credit file monitoring and identity theft assistance through Equifax. Activate the service using the instructions printed in your letter before the deadline. Enrollment is separate from, and does not waive, your right to pursue a claim.

The total number of affected individuals nationwide has not been publicly disclosed. A separate filing with the Texas Attorney General identified 319 Texas residents among those affected. Counts for other states have not been individually reported. This page will be updated as additional information becomes available.

Is there a deadline to take legal action? +
