What happened
Heart Care Centers of Illinois, S.C. ("HCCI") is a cardiology practice with offices in the Chicago area and a principal address at 13011 S. 104th Avenue, Suite 100, Palos Park, Illinois. According to the practice, an unauthorized actor gained access to an employee email account between August 22 and November 6, 2024. HCCI says the historic activity was identified on January 15, 2026, while it was investigating a separate and unrelated phishing attempt, and that it then engaged outside cybersecurity specialists to review what the account contained.
HCCI completed its review of the affected mailbox on June 11, 2026, and began mailing notification letters on July 10, 2026. Published reporting places the total number of individuals notified at 52,562. A separate notice filed with the Massachusetts Attorney General on August 24, 2026 covers Massachusetts residents and identifies full name and Social Security number as the data elements involved for that group. Roughly six months passed between discovery and the mailing of letters, and more than twenty months passed between the start of the unauthorized access and the first notice to affected individuals. Dapeer Law is evaluating whether that timeline is material to the claims available to notice recipients.
The categories of information involved vary by individual. HCCI's public notice states that the affected mailbox could have contained name, mailing address, phone and fax number, Social Security number, date of birth, driver's license or state identification number, payment card information, financial account information, passport number, and medical information such as diagnosis, treatment, prescription, health insurance, and provider details. Because both Social Security numbers and medical information may be involved, the exposure can raise concerns beyond financial fraud, including medical identity theft and fraudulent billing. HCCI states that it has no reason to believe there has been any identity theft, fraud, or misuse of information connected to the incident. A separate putative class action over the same incident was filed against HCCI in the Circuit Court of Cook County, Illinois on July 22, 2026 by other counsel, and Dapeer Law is independently evaluating claims on behalf of notice recipients.
What to do if you received a letter
Keep your notice letter
Do not discard it. Your letter contains the enrollment code for credit monitoring and is important evidence if you decide to participate in a lawsuit.
Enroll in the free 24-month credit monitoring
Enroll in the Epiq credit monitoring and identity restoration services offered in your letter before the October 31, 2026 deadline stated in the notice. Accepting this benefit does not waive your right to pursue legal action.
Place a fraud alert or credit freeze
Contact Equifax, Experian, and TransUnion to place a fraud alert or freeze on your file. Request a free weekly credit report from AnnualCreditReport.com, and use the FTC's IdentityTheft.gov recovery guide. Because Social Security numbers and driver's license numbers are among the elements reported, a credit freeze is worth considering in addition to a fraud alert.

I received a data breach letter from Heart Care Centers of Illinois. What should I do? +
Keep your notice letter, enroll in the free 24-month Epiq credit monitoring using the activation code in your letter before the October 31, 2026 deadline, and consider placing a fraud alert or credit freeze with Equifax, Experian, and TransUnion. Because medical information may be involved, also review explanations of benefits and provider statements for care you did not receive, and contact a data breach attorney about your rights.
Am I eligible to join a class action against Heart Care Centers of Illinois? +
Notice recipients are the most likely to qualify, including patients and current or former employees whose information was in the affected email account. Whether you can participate can depend on your state of residence, the categories of your data that were involved, and any documented losses or time you spent responding to the incident.
How much money could I receive from a class action lawsuit? +
Data breach class action recoveries vary significantly. Settlements typically range from a few hundred dollars for basic out-of-pocket losses to several thousand dollars for documented identity theft, with class size, damages, and negotiation all affecting the final amount. No payout is guaranteed, and this investigation has not yet resulted in a settlement.

HCCI reports that the affected mailbox could have contained name, mailing address, phone and fax number, Social Security number, date of birth, driver's license or state identification number, payment card information, financial account information, passport number, and medical information such as diagnosis, treatment, prescription, health insurance, and provider details. The Massachusetts filing identifies full name and Social Security number for the residents covered there. Check your individual letter for the elements that applied to you.
