What happened
Paradigm Healthcare Services ("Paradigm"), a San Francisco, California firm that provides Medi-Cal billing and consulting services as a third-party service provider under an agreement with the California Department of Health Care Services, states that it discovered unauthorized access to its network on October 13, 2025. The company reports that it took affected systems offline, engaged incident-response specialists, and notified law enforcement the same day. The investigation determined that an unauthorized party had access to the network between October 8 and October 15, 2025.
According to the notice, Paradigm confirmed on August 18, 2026 that certain protected health information was accessible to the unauthorized party. The company filed a copy of its notice with the California Attorney General and mailed letters to affected individuals on September 14, 2026. The notice lists the data elements that may have been viewed as name, date of birth, gender, and Medi-Cal member identification number.
