What happened

On or around April 7, 2026, Sanger Skilled Care, LLC d/b/a Cornerstone Care Center ("Cornerstone Care Center"), a skilled nursing facility headquartered at 2550 9th Street in Sanger, California, became aware of unauthorized activity involving one user account. According to the sample notice filed with the California Attorney General, the facility immediately took steps to contain the incident and engaged a third-party forensic firm to investigate.

On April 16, 2026, the forensic investigation determined that a limited amount of protected health information (PHI) maintained in the ordinary course of business may have been accessed by an unauthorized third party. Cornerstone Care Center began mailing notification letters on May 7, 2026, approximately one month after discovery, and is offering 12 months of complimentary credit monitoring through Cyberscout, a TransUnion company. The public sample notice does not itemize the specific categories of PHI involved and states there is no current indication that any information has been misused.
