What happened
On May 20, 2026, DentaQuest LLC detected unauthorized activity on its computer network. A subsequent investigation determined that an unauthorized party first gained access on May 17, 2026 and remained in the environment until May 20, 2026, when the intrusion was contained and reported to law enforcement.
DentaQuest began notifying affected individuals and filed notice with multiple state Attorneys General on July 16 and 17, 2026, roughly two months after discovery. According to those filings, 4,495,000 individuals were affected, including 3,973,000 Texas residents and 522,000 Massachusetts residents. The information involved may include names, Social Security numbers, addresses, dates of birth, health insurance information, and dental or vision health information such as provider names, diagnoses, treatments, and billing details. DentaQuest is offering affected members 24 months of free identity monitoring and restoration services through Kroll.
Because health insurance and dental or vision health data were involved, the incident may implicate the Health Insurance Portability and Accountability Act (HIPAA) as well as state data-breach and consumer-protection statutes. Whether DentaQuest exercised reasonable care in safeguarding member information is among the issues being evaluated.
