What happened
ExamOne, a Quest Diagnostics company, performs paramedical examinations and laboratory testing used by life insurance carriers to underwrite applications. According to a filing submitted to the Massachusetts Attorney General on July 10, 2026, ExamOne collected a consumer's specimens and paperwork in connection with a life insurance application, and those materials were never received by the testing laboratory as expected. The company began an internal review after the materials could not be accounted for.
ExamOne mailed written notice to the affected consumer on June 3, 2026 and reported the incident to Massachusetts regulators on July 10, 2026. The filing states that the paperwork may have contained names, addresses, dates of birth, driver's license numbers, the last four digits of Social Security numbers, telephone numbers, physical measurements, medical history and family medical history, medications, and physician names and addresses. ExamOne is offering two years of complimentary credit monitoring and identity protection services through Equifax Complete Premier, with an enrollment deadline of March 31, 2027, along with up to $1,000,000 in identity theft insurance and identity restoration support. The company states it has no indication that the information has been stolen or misused.
Life insurance application paperwork combines identifying data with detailed medical information, a pairing that carries risk beyond ordinary financial fraud. Driver's license numbers and partial Social Security numbers can support identity theft, while medical history, medications, and physician information can be used for medical identity theft or insurance fraud, and cannot be reissued the way a card number can. Whether ExamOne maintained reasonable safeguards for physical specimens and paperwork in transit, what happened to the missing materials, and how many people were ultimately affected are among the questions being evaluated.
What to do if you received a letter
Keep your notice letter
Do not discard it. Your letter contains the enrollment code for the Equifax credit monitoring and is important evidence if you decide to participate in a lawsuit.

Request a free weekly credit report from AnnualCreditReport.com, and use the FTC's IdentityTheft.gov recovery guide. Because medical information was involved, also review Explanation of Benefits statements from your health insurer and any correspondence from insurers you did not apply to.
