What happened
Firstsource Health Plans and Healthcare Services, LLC, a service provider to health insurance industry clients, identified a programming error on its document-upload platform that may have allowed unauthorized access to or viewing of certain personally identifiable information. The platform supports online uploads of documents, including claims and correspondence, submitted on behalf of providers and members for downstream data intake and processing. Firstsource received the affected information in connection with client engagements, including data related to individuals' health care through their health benefit plans.
After discovering the issue, Firstsource disabled the setting that allowed the information to be accessed, added protections to the related system, and strengthened access controls. The company engaged an independent forensic firm to review the platform and reported that, based on that investigation, there is no evidence any information was misused. Firstsource filed notice of the incident with the Massachusetts Attorney General on July 13, 2026 and is offering affected individuals two years of complimentary credit monitoring and identity protection services through Cyberscout, with enrollment required within 90 days of the notice date.
The information reported as potentially exposed is the stand-alone Social Security number, one of the most sensitive identifiers because it can be used to open fraudulent accounts, file false tax returns, or commit other forms of identity theft.
