What happened
On April 27, 2026, Green Accounting & Tax LLC, an accounting and tax preparation firm located at 100 E. Taylor Street, Suite 1, in Creston, Iowa, became aware of suspicious activity within its computer network. The firm reports that it immediately took steps to secure its systems and engaged a third-party team of forensic investigators to contain the activity and determine its nature and scope. That investigation concluded that certain files within the firm's environment may have been accessed or acquired by an unauthorized actor. Green Accounting & Tax also states that it notified law enforcement.
The firm then collected and reviewed the affected files to determine what categories of personal information they contained and to whom that information related. That review was completed on July 13, 2026. A National Change of Address search returned results on August 11, 2026, and the firm determined that the incident potentially affected 3,335 Iowa residents. Notice letters began going out by U.S. mail on August 12, 2026, and notice was filed with the Iowa Attorney General on August 13, 2026. According to that filing, the information potentially involved included first and last name, Social Security number, driver's license number, and financial account information. The template letter sent to consumers uses a variable field for the elements affected, so each recipient's own letter should specify which categories applied to them.
Green Accounting & Tax is offering complimentary identity protection and credit monitoring through Iris Identity Protection, which includes one-bureau credit monitoring from Equifax, identity monitoring, identity fraud insurance, and identity resolution services. Recipients must enroll themselves using the promo code printed in their letter, and the enrollment deadline is 90 days from the date of the letter. The public filing does not state how many months of coverage are being provided. Because tax and accounting firms hold concentrated financial records, the combination of Social Security numbers, driver's license numbers, and financial account information reported here can support tax refund fraud, fraudulent new accounts, and identity theft for years after an incident.
