What happened
Midwest Spine and Brain Institute (MSBI), a Stillwater, Minnesota spine and neurological care practice, disclosed a cybersecurity incident in which an unauthorized actor gained access to the network environment of one of the practice's third-party vendors. According to the notice, a forensic investigation determined that the actor accessed systems containing personal and protected health information on or about November 21, 2024. MSBI states that it immediately consulted with the vendor to confirm the threat was contained and the vendor's environment was secured, and that it engaged external cybersecurity professionals to investigate. Notice was filed with the Massachusetts Attorney General's Office and reported publicly on August 14, 2026.
The regulator filing reports 8 Massachusetts residents affected and identifies the categories of information involved as full name, medical information, and Social Security number. The interval between the reported access date and notification is a central issue in this investigation. Roughly 19 months passed between the November 2024 access date and the June 18, 2026 confirmation that the affected data contained consumer information, and roughly 21 months passed before notices were reported in August 2026. Several state breach notification statutes, and HIPAA's Breach Notification Rule, set outer limits on how long a covered entity or its business associate may take to notify affected individuals once a breach is discovered. MSBI is offering 12 months of complimentary single bureau credit monitoring, together with a credit report and credit score, through Cyberscout, with enrollment to be completed within 90 days of the date on the letter using the code printed in the notice.
Medical information paired with a Social Security number is a particularly sensitive combination. A Social Security number cannot be changed on request the way a payment card number can, and it supports new-account fraud, tax refund fraud, and fraudulent benefit claims for years after a breach. Medical information can be used for medical identity theft, in which treatment or prescriptions are obtained in a patient's name, corrupting the patient's own medical record. How the vendor's network was accessed, why the intrusion was not identified for roughly 19 months, what security requirements MSBI imposed on the vendor and how those requirements were monitored, and whether patients outside Massachusetts were also affected are among the questions being evaluated.

Because a Social Security number was reported as involved, consider requesting an IRS Identity Protection PIN and reviewing your Social Security earnings statement for employment you do not recognize. Because medical information was also involved, request and review an accounting of benefits from your health insurer for treatment you did not receive.
