What happened

Tapestry 360 Health, a community healthcare provider headquartered at 1301 W. Devon Ave. in Chicago, Illinois, began notifying patients on August 12, 2026 that protected health information stored on a technology vendor's system was copied by an unauthorized actor. According to the notice filed with the Massachusetts Attorney General's Office, the vendor, Aesto, reported that a limited portion of its Amazon Web Services environment was accessed without authorization between December 2 and December 18, 2025, and that files containing Tapestry 360 Health patient information were copied during that period.

The vendor engaged outside cybersecurity specialists, contained the incident, and performed a manual review of the affected documents. That review was completed on May 26, 2026, roughly two and a half months before notice letters were mailed on August 12, 2026. The filing reports the incident as affecting 24 Massachusetts residents and identifies the information involved as full name, date of birth, Social Security number, and medical record number. Tapestry 360 Health is funding a complimentary 24-month membership that includes single bureau credit monitoring, a single bureau credit report and credit score, and fraud resolution support. The total number of patients affected across all states, the name of the monitoring provider, and how the vendor's cloud environment was accessed are among the questions being evaluated.

Social Security numbers combined with dates of birth and medical record numbers are among the most damaging data sets to lose, because they support both new-account financial fraud and medical identity theft, where a stranger obtains care or prescriptions under a patient's identity. Unlike a payment card, a Social Security number and a medical record number cannot simply be reissued, and healthcare providers and their vendors have duties under HIPAA and state privacy laws to safeguard this information.

Contact Equifax, Experian, and TransUnion to place a fraud alert or freeze on your file. Request a free weekly credit report from AnnualCreditReport.com, and use the FTC's IdentityTheft.gov recovery guide. Because a medical record number was reported as involved, also review Explanation of Benefits statements and provider billing summaries for care you did not receive, and request a copy of your medical file if anything looks unfamiliar.
