You are a Class Member if ALL of the following apply:
- You are a living individual residing in the United States.
- Your private information was accessed in the February 2025 Bell Ambulance, Inc. data incident. This may include full names, Social Security numbers, dates of birth, driver's license numbers, financial information, medical records, health insurance information, or other protected health information.
- Excluded: Directors, officers, agents, and representatives of Bell Ambulance (except current or former employees whose private information was also accessed); the judge assigned to the case, their immediate family, and Court staff; and anyone who timely and validly opted out of the settlement.

What happened
In February 2025, Bell Ambulance, Inc. suffered a targeted cyberattack on its computer systems. The attackers accessed files containing the private information of patients, employees, and others, including full names, Social Security numbers, dates of birth, driver's license numbers, financial account information, medical treatment and diagnosis records, medical record numbers, health insurance information, full-face photographs, and other protected health information. Plaintiffs filed suit alleging that Bell Ambulance failed to implement adequate data security measures to protect this sensitive information.
Bell Ambulance denies the allegations and denies any wrongdoing or liability. The parties agreed to settle to avoid the cost, risk, and time of continued litigation.
How to file a claim

1. Confirm you are eligible
You must be a living U.S. resident whose private information was accessed in the February 2025 Bell Ambulance cyberattack. If Bell Ambulance mailed or emailed you a notice, you are likely a class member.

2. Choose your cash payment option
You can claim either Cash Payment A (up to $5,000 for documented out-of-pocket losses caused by the data incident, with supporting receipts or documentation) or Cash Payment B (a no-proof pro rata payment estimated at $90, which may be higher or lower depending on total valid claims filed). You may also enroll in two years of CyEx Medical Shield Complete medical records monitoring. You cannot claim both cash options.

3. File online or by mail
Submit the Claim Form online at belldataincidentsettlement.com/form/claim by June 29, 2026, or mail a paper Claim Form (postmarked by June 29, 2026) to: Bell Ambulance Data Incident Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958.

4. Receive your payment
Payments are issued after the Court grants final approval at the July 14, 2026 Final Approval Hearing and any appeals are resolved. The appeal process can take a year or more. No follow-up is required once your claim is submitted.

The Final Approval Hearing is scheduled for July 14, 2026 at 9:00 a.m. CT at the Milwaukee County Courthouse, 901 N. 9th Street, Milwaukee, WI 53233. The Court may move the hearing, so check BellDataIncidentSettlement.com for updates before traveling.
Where the money is going
This is a non-reversionary common fund. Attorneys' fees (up to 33.33%), service awards ($2,500 per class representative), and notice and administration costs come out of the $2 million before payments are calculated for class members.
All amounts listed above are pending and subject to final court approval at the July 14, 2026 Final Approval Hearing. The Court may award less than the amounts requested. The Cash Payment B pro rata amount of approximately $90 per claimant will vary based on the total number of valid claims filed.
Common questions
How much money will I receive?
You have two options. Cash Payment A pays up to $5,000 if you submit receipts or documentation proving out-of-pocket losses caused by the data breach (such as identity theft costs, credit monitoring fees, or ID replacement expenses). Cash Payment B is a simpler no-proof option estimated at $90, though the final amount may be higher or lower depending on how many people file valid claims. You can only choose one cash option per claim.
Do I need to submit proof of purchase?
Cash Payment B (estimated $90) requires no documentation at all. Cash Payment A (up to $5,000) requires receipts or other documentation showing your actual out-of-pocket losses were caused by the Bell Ambulance data incident. Personal declarations or affidavits alone are not enough for Cash Payment A, though they may be submitted alongside other documentation.
What if I didn’t receive a notice?
Even if you did not receive a notice, you may still be a class member if your private information was accessed in the February 2025 Bell Ambulance data incident. Contact the Settlement Administrator at 1-844-804-2567 or info@BellDataIncidentSettlement.com to confirm your eligibility and request a Claim Form.
Does staying in the class affect my right to sue later?
If you remain in the settlement class and do not opt out, you give up the right to sue Bell Ambulance separately over the claims resolved by this settlement. To preserve your right to sue independently, you must mail an opt-out request postmarked by June 15, 2026, to: Bell Ambulance Data Incident Settlement, ATTN: Exclusion Request, P.O. Box 25226, Santa Ana, CA 92799-9958.
When will payments be sent out?
The Court will hold a Final Approval Hearing on July 14, 2026. If the settlement is approved and no appeals are filed, payments are expected to be distributed sometime after that date. Appeals can extend the timeline by a year or more. Once you submit your claim, no further action is required.
