- You are an individual residing in the United States whose PII or PHI was identified as at issue in the Data Incident discovered by CAM in October 2024.
- This includes people who received notice of the Data Incident from Cardiology Associates of Mobile.
- Excluded: Anyone who opts out of the settlement by the November 30, 2026 deadline.

What happened
The lawsuit concerns a data security incident at Cardiology Associates of Mobile, Inc. (CAM), a cardiology practice in Mobile, Alabama. On or about October 22, 2024, unauthorized third parties accessed CAM's computer systems and data, resulting in access to personally identifiable information (PII) and personal health information (PHI). Teklinks, Inc. and its affiliates are included in the settlement as Released Parties.
The Defendant denies all allegations and any wrongdoing, and the Court has not decided who is right. The parties agreed to settle to avoid the cost, risk, and delay of continued litigation. The $1,250,000 Settlement Fund pays class member benefits as well as notice and administration costs, taxes, attorneys' fees and costs, and any service awards.

1. Confirm you are eligible
You qualify if you live in the United States and your PII or PHI was identified as at issue in the Data Incident that CAM discovered in October 2024, including if you received a notice about it. If you are unsure, call the Settlement Administrator at (833) 453-3702.

2. Choose your benefits
Class members can claim one or both cash benefits: reimbursement of up to $4,000 for documented out-of-pocket losses, and a pro rata cash payment that requires no documentation. Everyone can also claim 24 months of credit monitoring. The pro rata cash amount depends on the number of valid claims and the money left after fees and costs.

3. File online or by mail
Submit the Claim Form online at the official claim portal by 11:59 p.m. CT on December 29, 2026, or mail a paper Claim Form postmarked on or before that date to Settlement Administrator - 83473, c/o Kroll Settlement Administration LLC, P.O. Box 225391, New York, NY 10150-5391. The online form asks for the Class Member ID printed on your notice. Out-of-pocket loss claims require supporting documentation. Notice documents, the Settlement Agreement, and the Preliminary Approval Order are posted at CardiologyDataIncident2.com.

4. Receive your benefits
Cash payments and credit monitoring enrollment are provided after the Court grants final approval at the January 29, 2027 Final Approval Hearing and any appeals are resolved. No follow-up is required once your claim is submitted.

The Final Approval Hearing is scheduled for Friday, January 29, 2027 at 10:00 a.m. CT in Courtroom 6500 at the Mobile County Courthouse, 205 Government Street, Mobile, AL. The Court will also consider Class Counsel's request for attorneys' fees and costs and any service awards, which are paid from the Settlement Fund. Opt-out requests and objections must be postmarked by November 30, 2026. The Court may move the hearing, so check CardiologyDataIncident2.com for updates before traveling.
Where the money is going
Cardiology Associates of Mobile will pay $1,250,000 into a Settlement Fund. The fund covers benefits for class members who submit valid claims, notice and administration costs, taxes, attorneys' fees and costs, and any service awards. Because cash payments are pro rata, the final amount per claimant depends on how many valid claims are filed.
All amounts listed above are pending and subject to final court approval at the Final Approval Hearing. The Court may award less than the amounts requested. Out-of-pocket reimbursements and cash payments may be adjusted pro rata based on the number of valid claims.
Common questions
How much money will I receive?
You can claim reimbursement of up to $4,000 for documented out-of-pocket losses and/or a pro rata cash payment, plus 24 months of credit monitoring. The cash amount is not fixed and depends on the number of valid claims and the money left in the fund after fees, costs, and administration expenses.
Do I need to submit proof of purchase?
The pro rata cash payment requires no documentation. Out-of-pocket loss claims require documentation supporting the loss. You still need to submit a valid Claim Form, and the online form asks for the Class Member ID on your notice.
What if I didn’t receive a notice?
The class includes individuals whose information was identified as at issue in the Data Incident, including those who received notice. The online Claim Form asks for a Class Member ID from the notice. If you believe you were affected but did not get a notice or Class Member ID, call Kroll Settlement Administration at (833) 453-3702 before the December 29, 2026 deadline.
Does staying in the class affect my right to sue later?
If you stay in the settlement, you give up the right to sue, continue to sue, or be part of any other lawsuit against Cardiology Associates of Mobile and the other Released Parties, including Teklinks, Inc. and its affiliates, over the legal claims resolved by this settlement, whether or not you file a claim. To keep those rights, you must opt out by November 30, 2026.
When will payments be sent out?
Benefits are distributed after the Court grants final approval at the January 29, 2027 Final Approval Hearing and after any appeals are resolved. An appeal can add a year or more to that timeline.
