Onsite Women's Health Data Breach Class Action Settlement
Onsite Mammography, LLC, d/b/a Onsite Women's Health, agreed to a $2,525,000 settlement after an October 2024 incident in which an unauthorized third party accessed a single employee's email account containing patients' personal and health information. Class Members who were notified that their information may have been affected can claim reimbursement for documented losses up to $5,000, a pro rata cash payment, and three years of free credit and medical data monitoring, by August 11, 2026.

- You are an individual residing in the United States whose Private Information may have been impacted by the October 2024 email account incident, including anyone who received written notice from Onsite about the Incident.
- No purchase or proof is required for the Pro Rata Cash Payment; documentation is only needed if you're claiming reimbursement for specific out-of-pocket losses.
- Excluded: Onsite Mammography, LLC and its current or former parents, subsidiaries, affiliates, and their officers and directors.
- Excluded: The judges presiding over the case and members of their families.

What happened
In October 2024, an unauthorized third party gained access to a single Onsite Mammography, LLC employee's email account. An investigation determined the account contained files that may have included Settlement Class Members' personally identifiable information (PII) and protected health information (PHI).
Onsite denies any wrongdoing and denies that it has any liability, but agreed to settle the lawsuit on a classwide basis to avoid the cost, risk, and time of continued litigation.

You qualify if you received notice that your Private Information may have been impacted by the October 2024 email account incident involving Onsite Mammography, LLC, d/b/a Onsite Women's Health.

2. Choose your benefits
Claim reimbursement for documented out-of-pocket losses up to $5,000, a Pro Rata Cash Payment (amount depends on how many valid claims are filed), or both. You can also enroll in three years of free Credit and Medical/Healthcare Data Monitoring and Insurance Services (CMIS), including $1 million in identity theft insurance, regardless of which cash option you choose.

3. File online or by mail
Submit the Claim Form online at onsitesettlement.pnclassaction.com by August 11, 2026, or mail a paper Claim Form (postmarked by August 11, 2026) to: Onsite Settlement Administrator, P.O. Box 3868, Baton Rouge, LA 70821.

4. Receive your payment
Payments are issued after the Court grants final approval at the September 9, 2026 Fairness Hearing and any appeals are resolved. The appeal process can take a year or more. No follow-up is required once your claim is submitted.

Where the money is going
This is a non-reversionary common fund. Attorneys' fees (up to one-third of the fund), expenses (up to $30,000), service awards, and settlement administration costs come out of the $2,525,000 fund before payments for Out-of-Pocket Losses and Pro Rata Cash Payments are calculated.
All amounts listed above are pending and subject to final Court approval at the September 9, 2026 Fairness Hearing. The Court may award less than the amounts requested. Reimbursement claims for Out-of-Pocket Losses are paid first (up to $5,000 per person), and the Pro Rata Cash Payment is calculated from whatever remains, so it may be increased or decreased depending on how many valid claims are filed.

There is no set per-person Pro Rata Cash Payment amount; it depends on how many valid claims are filed and will be calculated from whatever remains in the $2,525,000 fund after attorneys' fees and costs, service awards, and administration expenses. Separately, you can be reimbursed up to $5,000 for documented out-of-pocket losses tied to the Incident.
Do I need to submit proof of purchase?
Documentation is required only if you're claiming reimbursement for specific Out-of-Pocket Losses (your name and address, supporting documentation for the cost or loss, and a brief description if the nature of the loss isn't clear from the documentation alone). No proof is required to claim the Pro Rata Cash Payment or to enroll in credit and medical data monitoring.
What if I didn’t receive a notice?
Onsite's records were used to identify and notify Settlement Class Members by mail, including a postcard with a tear-off Claim Form. If you believe your information was affected by the Incident but did not receive a notice, call the Settlement Administrator at 1-855-389-9238 before the claim deadline to confirm your status.
Does staying in the class affect my right to sue later?
Yes. Unless you exclude yourself (opt out) by submitting a request online or by mail postmarked by July 13, 2026, you will remain in the Settlement and give up the right to sue Onsite over the claims resolved by this Settlement. When will payments be sent out?
The Court will hold the Final Approval Hearing on September 9, 2026 at 12:00 p.m. ET. Payments are issued after the Court grants final approval and after any appeals are resolved, which can take a year or more. No action is required after you submit a valid Claim Form.
