Do you qualify?
You are a Class Member if ALL of the following apply:
- You are a U.S. resident whose Private Information was confirmed impacted in the April 2022 OTP Data Incident. This is the Monetary Settlement Class, which can claim cash benefits.
- The $75 Alternative Cash Payment requires no documentation or attestation of time spent.
- U.S. residents who were sent notice of potential impact, but whose impact could not be confirmed, are in the Injunctive Relief Settlement Class. That class receives OTP's security commitments, not cash.
- Excluded: OTP's officers and directors, entities OTP controls, its affiliates and legal representatives, the assigned judges and their staff, and anyone who opts out by October 16, 2026.

What happened
The lawsuit concerns a cybersecurity incident at OneTouchPoint Corp. (OTP) in April 2022 (the "Data Incident"). According to the Settlement Agreement, OTP is a mailing and printing services vendor for companies in the healthcare sector, and the information involved included names, addresses, Social Security numbers, member IDs, dates of birth, health insurance information, and other medical information. The people affected include current and former patients and subscribers of OTP's clients, as well as OTP employees.
OTP denies all claims and the Court has not decided who is right. The parties agreed to settle to avoid the costs and risks of continued litigation. This is a claims-made settlement, so there is no fixed common fund. OTP pays valid claims, and it pays notice and administration costs separately from the benefits to class members. OTP also agreed to enhance security in relevant systems for at least five years.
How to file a claim

1. Confirm you are eligible
Cash benefits are available to Monetary Settlement Class Members, meaning U.S. residents whose Private Information was confirmed impacted in the April 2022 OTP Data Incident. If you are unsure which class you are in, call the Settlement Administrator at 866-996-3808.

2. Choose your benefits
You can claim reimbursement of ordinary losses up to $500, such as bank fees, long-distance phone charges, data charges, and postage, and extraordinary losses up to $5,000, such as proven monetary losses, professional fees, and credit repair fees. Documentation is required for losses. You can also claim lost time at $25 per hour for up to 4 hours ($100) by attestation. Instead of losses and lost time, you can choose a one-time $75 Alternative Cash Payment with no documentation required. The Claim Form also offers two years of single-bureau credit monitoring with $1 million in fraud protection.

3. File online or by mail
Submit the Claim Form online at the Analytics Consulting claim portal by November 16, 2026. The online portal asks for the Claim Number and PIN from your notice. If you do not have a Claim Number, you can download the paper Claim Form from the Important Case Documents page and mail it, postmarked by November 16, 2026, to OTP Data Incident Settlement, c/o Analytics Consulting LLC, PO Box 2005, Chanhassen, MN 55317-2005. Notice documents and the Settlement Agreement are posted at OTPDataIncident.com.

4. Receive your payment
Payments are issued after the Court grants final approval at the November 18, 2026 Final Approval Hearing and any appeals are resolved. Claims are reviewed for completeness and plausibility, and the Settlement Administrator may contact you if more information is needed.

The Final Approval Hearing is scheduled for November 18, 2026 at 10:00 a.m. at the Waukesha County Circuit Court, 515 W. Moreland Blvd., Waukesha, WI 53188. The Court will also consider Class Counsel's request for attorneys' fees and costs of up to $1,500,000, paid by OTP, and service awards of $1,000 for each Class Representative. Opt-outs and objections are due October 16, 2026, and claims are due November 16, 2026. The Court may move the hearing, so check OTPDataIncident.com for updates before attending.
Where the money is going
This is a claims-made settlement with no fixed common fund. OTP pays each valid claim according to the benefits selected, and it pays notice and settlement administration costs separately from the relief provided to class members. Attorneys' fees, costs, and service awards are subject to Court approval.
All amounts listed above are pending and subject to final court approval at the Final Approval Hearing. The Court may award less than the amounts requested. Benefit amounts are per valid claim and are paid only after final approval and any appeals.
Common questions
How much money will I receive?
It depends on what you claim. Monetary Settlement Class Members can claim up to $500 in ordinary losses, up to $5,000 in extraordinary losses, and up to $100 for lost time, or a one-time $75 Alternative Cash Payment instead of losses and lost time. Two years of credit monitoring is also offered on the Claim Form.
Do I need to submit proof of purchase?
Not for the $75 Alternative Cash Payment. Lost time is claimed by attestation. Ordinary and extraordinary losses require supporting documentation showing unreimbursed expenses related to the Data Incident.
What if I didn’t receive a notice?
The online portal asks for the Claim Number and PIN from your notice. If you do not have a Claim Number, the Settlement Agreement allows you to submit a paper Claim Form for consideration. Does staying in the class affect my right to sue later?
If you are a Monetary Settlement Class Member and stay in the settlement, you give up the right to sue OTP over the claims released by this settlement, whether or not you file a claim. Injunctive Relief Settlement Class Members release only claims for injunctive or declaratory relief. To keep your rights, you must opt out by October 16, 2026.
When will payments be sent out?
Payments come after the Court grants final approval at the November 18, 2026 Final Approval Hearing and after any appeals are resolved. An appeal can add a year or more to that timeline.
