Lucent Health Solutions, LLC agreed to a settlement with an aggregate cap of $1,950,000 on cash payments over a targeted cyberattack on its computer systems in October 2023 that may have exposed private information, including full names, dates of birth, and health, dental, and vision policy, member, group, and plan numbers. Class members whose information was implicated can enroll in three years of free identity theft protection and claim a cash payment of up to $5,500 for documented losses, or a no-proof $80 flat payment.

- Your private information was implicated in the October 2023 data security incident involving Lucent Health Solutions, LLC, including if Lucent Health sent you an individual notification letter about the Data Incident.
- Excluded: Lucent Health and its officers, directors, and related companies; governmental entities; the judge presiding over the case, and the judge's family and staff; and anyone who validly excludes themselves from the Settlement.

What happened
Plaintiff Royal Corralejo filed this class action against Lucent Health Solutions, LLC after a targeted and isolated cyberattack on Lucent Health's computer systems in October 2023 (the Data Incident). Certain files that may have contained private information, including full names, dates of birth, and health, dental, and vision policy, member, group, and plan numbers, may have been accessed.
Lucent Health denies that it did anything wrong, and the Court has not decided who is right. The parties agreed to settle the case, Corralejo v. Lucent Health Solutions, LLC, Case No. 25C2679, pending in the Circuit Court for Davidson County, Tennessee, to avoid the costs, risks, and uncertainties of continued litigation.

1. Confirm you are eligible
You must be someone whose private information was implicated in the October 2023 Lucent Health data incident, including anyone who received an individual notification letter from Lucent Health about it.

2. Choose your payment option
All Class Members can enroll in three years of CyEx Medical Shield Complete at no cost. You may also claim one cash payment option: up to $550 for documented ordinary expenses, up to $5,500 for documented fraud or identity theft losses, up to $125 for five hours of lost time at $25 per hour, or a no-proof $80 flat payment. Cash payments are subject to an aggregate cap of $1,950,000 and will be reduced pro rata if claims exceed that cap.

3. File online or by mail
Submit the Claim Form online at lhdatabreachsettlement.com/form/claim by September 5, 2026, or mail a paper Claim Form (postmarked by September 5, 2026) to: Lucent Health Data Incident Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958.

4. Receive your payment
Payments are issued after the Court grants final approval at the September 9, 2026 Fairness Hearing and any appeals are resolved. The appeal process can take a year or more. No follow-up is required once your claim is submitted.

The Fairness Hearing is scheduled for September 9, 2026 at 9:30 a.m. Central Time before the Circuit Court for Davidson County, Tennessee at the Metropolitan Courthouse, 1 Public Square, Nashville, TN 37201. The Court may move the hearing, so check LHDataBreachSettlement.com for updates before traveling.
Where the money is going
This is a claims-made settlement, not a common fund. There is an aggregate cap of $1,950,000 on cash payments to Class Members; if total valid cash claims exceed that cap, individual payments are reduced pro rata. Attorneys' fees, costs, and the service award are paid separately by Lucent Health and do not reduce the $1,950,000 available to Class Members for cash payments.
All amounts listed above are pending final court approval at the Fairness Hearing. The Court may award less than the amounts requested. If valid cash payment claims exceed the $1,950,000 cap, individual payments will be reduced on a pro rata basis.
Common questions
How much money will I receive?
Class Members may claim one cash payment option: up to $550 for documented ordinary out-of-pocket expenses, up to $5,500 for documented losses from fraud or identity theft, up to $125 for lost time (five hours at $25 per hour), or a flat $80 no-proof Alternative Cash Payment. All cash payments are subject to an aggregate cap of $1,950,000; if total valid claims exceed that cap, payments will be reduced pro rata so they add up to $1,950,000.
Do I need to submit proof of purchase?
Ordinary and extraordinary out-of-pocket expense claims require documentation such as receipts, along with any notes explaining how the expenses relate to the Data Incident (notes or explanations alone are not enough to make a valid claim on their own). The Lost Time claim does not require receipts but must describe tasks related to the Data Incident, such as changing passwords or researching the incident. The $80 Alternative Cash Payment requires no proof or explanation at all.
What if I didn’t receive a notice?
The Notice defines the Class as everyone whose private information was implicated in the Data Incident, including anyone who received an individual notification letter from Lucent Health. If you believe your information was involved but are unsure whether you received a notice, contact the Settlement Administrator, Simpluris, at 1-833-421-7240 or info@LHDataBreachSettlement.com to confirm your status.
Does staying in the class affect my right to sue later?
Unless you exclude yourself by August 21, 2026, you will remain in the Settlement and give up the right to sue, continue to sue, or be part of another lawsuit against Lucent Health over the claims resolved by this Settlement. To opt out, mail a written Request for Exclusion postmarked (or emailed) by August 21, 2026 to the Settlement Administrator. When will payments be sent out?
The Court will hold the Final Approval Hearing on September 9, 2026. Payments will only be issued after the Court grants final approval and any appeals are resolved, which can take a year or more. No action is required after a valid Claim Form is submitted; check LHDataBreachSettlement.com for updates.
