Arch Health Partners, Inc. d/b/a Palomar Health Medical Group agreed to a $3.1 million settlement over a data incident on its systems between April 23 and May 5, 2024. Anyone whose private information was accessed, acquired, disclosed, or compromised in the incident can claim two years of credit monitoring plus either an estimated $60 cash payment with no documentation or reimbursement of up to $5,000 in documented losses. - Your Private Information was accessed, acquired, disclosed, or compromised in the Palomar Health Medical Group Data Incident.
- The incident occurred between April 23, 2024 and May 5, 2024.
- Every class member may claim 2 years of credit monitoring, and the estimated $60 Cash Payment B requires no documentation.
- Excluded: Directors, officers, and agents of the Defendant, governmental entities, and the assigned judge, the judge's immediate family, and court staff.
- Excluded: Anyone who opts out of the settlement by the October 7, 2026 deadline.

What happened
The lawsuit concerns unauthorized access to or acquisition of private information on Palomar Health Medical Group's systems between April 23, 2024 and May 5, 2024. According to court filings, the information involved may have included names, addresses, dates of birth, Social Security numbers, medical and treatment information, health insurance information, and in some cases payment card details and account credentials. The Court's preliminary approval order describes a class of approximately 1,132,116 people.
Palomar Health Medical Group denies that it did anything wrong, and the Court has not decided who is right. The parties agreed to settle to avoid the cost, risk, and delay of continued litigation. According to the class notice, since the incident the Defendant has deployed endpoint detection and response tools, added new backup storage capabilities, and enhanced network monitoring at its own cost.

2. Choose your benefits
You may choose one cash option. Cash Payment A reimburses documented losses up to $5,000. Cash Payment B is an alternative cash payment estimated at $60 and requires no documentation. You cannot claim both A and B. You may also select 2 years of single-bureau credit monitoring with $1,000,000 in identity fraud insurance and dark web monitoring in addition to either cash option. Cash payments may be adjusted up or down pro rata depending on the number of valid claims.

3. File online or by mail
Submit a claim online at PHMGDataSettlement.com by October 22, 2026, or mail a paper Claim Form postmarked by that date to: Palomar Health Data Incident Settlement, PO Box 60, East Brunswick, NJ 08816-9998. The paper Claim Form asks for your Notice ID if known, so have your notice handy. Cash Payment A claims need reasonable third-party documentation such as receipts, phone records, or emails; a personal declaration alone is not enough.

4. Receive your benefits
Payments are issued after the Court grants final approval at the November 6, 2026 Final Approval Hearing and any appeals are resolved. The Claim Form lists PayPal, Venmo, Zelle, virtual prepaid card, and paper check as payment options.

The Final Approval Hearing is scheduled for November 6, 2026 at 10:30 a.m. in Department C-64 (Courtroom SD-64), Hall of Justice, 330 W. Broadway, Fourth Floor, San Diego, CA 92101. The Court will also consider Class Counsel's request for attorneys' fees of up to one-third of the Settlement Fund plus costs, and $2,500 for each of the five Class Representatives. Objections and exclusion requests must be postmarked by October 7, 2026. The Court may move the hearing, so check PHMGDataSettlement.com for updates before traveling.
Where the money is going
Palomar Health Medical Group will pay $3.1 million into a Settlement Fund. The fund pays notice and administration costs, court-approved attorneys' fees, costs, and service awards first, then credit monitoring, then Cash Payment A claims, then Cash Payment B claims. Funds remaining after the payment window are proposed to go to the Electronic Privacy Information Center (EPIC) as cy pres.
All amounts listed above are pending and subject to final court approval at the Final Approval Hearing. The Court may award less than the amounts requested. Cash payments are estimates and may be adjusted pro rata based on the number of valid claims.

Cash payments may be increased or reduced pro rata depending on how many valid claims are filed.
Do I need to submit proof of purchase?
It depends on the option. Cash Payment B requires no documentation. Cash Payment A requires reasonable documentation, such as telephone records, emails, or receipts, and a personal declaration alone is not sufficient. Credit monitoring requires no documentation.
What if I didn’t receive a notice?
You may still be a class member if your information was involved in the incident. The paper Claim Form asks for a Notice ID only if known. If you believe you were affected but did not receive a notice, contact Angeion Group at 1-844-440-4203 or info@PHMGDataSettlement.com before the October 22, 2026 deadline.
Does staying in the class affect my right to sue later?
If you stay in the settlement, you give up the right to sue, continue to sue, or be part of any other lawsuit against Palomar Health Medical Group and the other Released Parties over the claims resolved by this settlement, whether or not you file a claim. To keep those rights, you must opt out by October 7, 2026.
When will payments be sent out?
Benefits are distributed after the Court grants final approval at the November 6, 2026 hearing and after any appeals are resolved. Processing time also depends on the number of claims. An appeal can add a year or more to that timeline.
