Memorial Health / MemorialCare Pixel Tracking Class Action Settlement
Memorial Health Services, which operates as MemorialCare, agreed to a $750,000 class action settlement over allegations that it used the Meta Pixel and other tracking, analytics, and advertising technologies on its MyChart patient portal to disclose patients' personally identifiable information to third parties without consent, in violation of the California Invasion of Privacy Act. Individuals who accessed the Memorial Health Services MyChart portal between March 7, 2022, and July 8, 2022, can file a claim for a pro rata cash payment from the net settlement fund.

- Excluded: Memorial Health Services and its affiliates, parents, subsidiaries, officers, and directors; and the judge(s) presiding over this matter and their clerks. (This exclusion does not apply to otherwise eligible Memorial employees who do not timely opt out.)

What happened
The lawsuit alleges that Memorial Health Services used the Meta Pixel and other tracking, analytics, and advertising technologies on its MyChart patient portal in a way that disclosed patients' personally identifiable information to third parties without consent, in violation of the California Invasion of Privacy Act, Cal. Pen. Code Section 631(a). Plaintiff Michelle Valladolid filed the case, Valladolid v. Memorial Health Services, Case No. 23STCV05059, in the Superior Court of the State of California, County of Los Angeles, on behalf of everyone who accessed the MyChart portal between March 7, 2022, and July 8, 2022.
Memorial Health Services denies all of Plaintiff's claims and maintains it did nothing wrong, and the Court has not decided who is right. The parties agreed to settle to avoid the expense, burden, and uncertainty of continuing the litigation.
How to file a claim

1. Confirm you are eligible
You must have accessed the Memorial Health Services MyChart patient portal at any time between March 7, 2022, and July 8, 2022. Many Class Members were sent a notice by mail or email with a unique Login ID and PIN.

2. Locate your Login ID and PIN
You'll need the unique Login ID and PIN from the notice you received by mail or email to file online. If you didn't receive one but believe you qualify, call the Settlement Administrator at 1-833-421-7255 to verify your identity.

3. File online or by mail
Submit the Claim Form online at MHSPixelSettlement.com/form/claim by 11:59 p.m. CT on August 21, 2026, or mail a paper Claim Form (postmarked by August 21, 2026) to: MHS Pixel Privacy Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799.

4. Receive your payment
Payments are issued after the Court grants final approval at the September 17, 2026 Final Approval Hearing and any appeals are resolved. Payment is made by check, PayPal, Venmo, or Zelle; checks expire 180 days after issuance. No follow-up is required once your Claim Form is submitted.

Where the money is going
This is a non-reversionary common fund. Attorneys' fees and costs, the Class Representative service award, and settlement administration costs are paid from the $750,000 Settlement Fund before payments to Class Members are calculated.
All amounts above are pending final court approval at the September 17, 2026 Fairness Hearing. The Court may award less than the amounts requested. Each Settlement Class Member who submits a valid, timely Claim Form receives an equal pro rata share of whatever remains after deductions. Any funds not distributed to Class Members will go to the Legal Aid Foundation of Los Angeles.

How much money will I receive?
Each Settlement Class Member who submits a valid Claim Form receives a pro rata share of the net settlement fund. There is no fixed per-person amount; it depends on how many valid claims are filed. You must provide the unique Login ID and PIN from your mailed or emailed notice to file.
Do I need to submit proof of purchase?
You must provide the unique Login ID and PIN from the notice you received by mail or email. If you did not receive this information but believe you are a Class Member, call the Settlement Administrator at 1-833-421-7255 to verify your identity and get instructions on how to file.
What if I didn’t receive a notice?
Call the Settlement Administrator at 1-833-421-7255 if you believe you meet the eligibility criteria (accessed the Memorial Health Services MyChart portal between March 7, 2022, and July 8, 2022) but did not receive a notice.
Does staying in the class affect my right to sue later?
If you do not exclude yourself by August 21, 2026, you remain in the Settlement Class and release your claims against Memorial Health Services as described in the Settlement Agreement, whether or not you file a claim. To opt out, mail or email a signed letter with your name, address, the case name and number, and a statement that you wish to be excluded to: MHS Pixel Privacy Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799, postmarked no later than August 21, 2026.
